Use this form if you would like a Focus in Pix artist to create the above card for you. Simply fill out the form below and upload picture file(s). We will email you a proof to look at before we proceed with printing your order.

When you request artist layout, you will be charged a Standard Artist Fee of $15 which includes 15 minutes of artist time. This fee is in addition to our card prices. If your request for customization goes beyond our standard customization, you will be notified via email or phone before we start your project.
